2009 Health Screening Program

HealthQuest offers free health screenings to all eligible members in multiple locations statewide. Health screenings can provide you with important health information including:
- Total cholesterol, HDL, LDL and total cholesterol to HDL ratio
- Triglycerides and glucose
- Systolic and diastolic blood pressure
- Height, weight and body mass index (BMI)
- Health information to fill out your Personal Health Assessment (PHA)
It is necessary to fast for nine hours (no food or drink) before your appointment. While fasting, only water and black coffee may be consumed (no cream or sugar). Take any medications according to your normal routine. You may also brush your teeth.
You may call your health coach or primary care physician at anytime to learn more about what your screening results mean to your overall health.
All eligible participants who complete the online Personal Health Assessment (PHA) using results obtained from a health screening event or doctor visit will earn a $50 gift card.

To Schedule an Appointment:
- Check the full schedule of events listed below to find out when a screening is offered in your area.
- Go to the Dialog Center at www.thedialogcenter.com/healthquest to make an appointment online. If you have ever registered at this site, your login will be the same as you had before.
- If you do not have internet access or need assistance making an appointment, please call toll-free 1-888-275-1205 and select option zero. (An email address is required in order to receive an appointment confirmation and reminder.)
Forgot your Login ID?
- From the Dialog Center, you can request your login ID by calling the Help Desk toll-free at 1-888-275-1205 and select option zero.
- If you choose to re-register as a first time user, you will not have access to information previously stored on your account such as your PHA actiona plan from last year.
Forgot your Password?
- From the Dialog Center, click on "show password hint" to display the hint you created when you registered.
- If you still do not remember your pasword, then you will be instructed to fill in the "answer to your secret question" field below your passoword hint. When you answer your secret question correctly, your password will be e-mailed to you at the email address you provided when you registered. This may take up to 15 minutes.

Additional Information
- The appoinment system is available form March 16 through September 10, 2009.
- The appointment system closes at noon five business days prior to event date.
- An appointment is required to attend a health screening event. Walk-ins will not be accepted. It is suggested to arrive at the event 10 minutes before your scheduled appointment time.
- If you are making an appointment for an event in a buliding other than your place of employment, keep in mind that some locations have secured access by employee badge prior to business hours.
- Be sure to request ADA accommodations when making an appointment.
