Who is eligible to participate?

The call center representative will help verify your eligibility and help you create a username and password so that you can access all programs. Be sure to write this information down for later access to the programs.

Health Screenings are available during work time at 51 locations statewide

Schedule a health screening appointment (or use a recent screening from your doctor--within past 6 months)
Screening includes total cholesterol, triglycerides, height, weight, blood pressure, and glucose.
Use your screening results to complete the online Personal Health Assessment (PHA) and receive information in the mail about how to redeem your $50 gift card.
Fast for 9 hours prior to your screening. While fasting, only water and black coffee may be consumed (no cream or sugar).
