To Complete the Online PHA:

• Attend a health screening event offered in your area or use test results obtained form your doctor. You must have your test results before you begin the PHA; it cannot be edited later.
  
  
• Go to the Dialog Center at www.thedialogcenter.com/healthquest to do the PHA. If you have ever registered at this site, your login will be the same as you had before. 
  

Forgot your Login ID?

• From the Dialog Center, you can request your login ID by calling the Help Desk toll-free at 1-888-275-1205 and select option zero.
  
  
• If you choose to re-register as a first time user, you will not have access to information previously stored on your account such as your PHA action plan from last year.

Forgot your Password?

• From the Dialog Center, click on "show password hint" to display the hint you created when you registered.
  
  
• If you still do not remember your password, then you will be instructed to fill in the "answer to your secret question" field below your password hint. When you answer your secret question correctly, your password will be e-mailed to you at the email address you provided when you registered. This may take up to 15 minutes.
