A HealthQuest Health Coach can help you create a personalized plan to help you better manage your health. Talk to your health coach today to get information and support for:

• Healthy Lifestyle Programs: Healthy eating, weight management, tobacco cessation, and stress management
• Significant medical issues such as surgery or chronic conditions like diabetes, congestive heart failure, asthma or COPD
• Clear and concise, evidence-based information on available surgery and treatment options based on your needs and values
• Working with you to better communicate with your doctor

Health coaches can send printed materials and relevant videotapes to your home on conditions such as heart disease, breast cancer and more.   Call toll-free 1-888-275-1205 to speak to a health coach—a specially trained health professional—anytime, day or night.

Health Screenings

• Total Cholesterol, triglycerides and glucose numbers.
• Blood pressure and Body Mass Index (BMI) numbers.

Personal Health Assessment (PHA) Available all year!

The Personal Health Assessment (PHA) online tool can help employees and eligible spouses and dependents take an active role in managing their health. Complete your PHA and receive a $50 gift card incentive. Follow these 3 easy steps:

1. Attend a HealthQuest on-site screening or use a recent screening from your primary care medical home provider (past 6 months) to determine your current health information (total cholesterol, triglycerides, height, weight, resting blood pressure and blood glucose).
2. Use the information from your health screening to take the online Personal Health Assessment (PHA) .
3. Complete the PHA and receive information in the mail (within 7-10 days) about how to redeem your $50 incentive award.

Drug Benefits and Wellness Programs for Tobacco Control and Weight Management

To participate:

1. Call a Health Coach and ask to be enrolled in the Healthy Lifestyle Smoking Cessation or Weight Management program.  Call toll-free 24/7/365:  1.888.275.1205
2. Contact your primary care medical home to receive a prescription for covered medications. 
3. Present your Caremark ID card at the pharmacy when your prescription is filled. That's it!

Who is Eligible to Participate in the New HQ Programs?

• Employees and spouse/dependents 18 years or older who are enrolled in the state medical plan
• Benefits eligible employees who have waived coverage in the medical plan
• Retirees and spouse/dependents age 18 or older who are enrolled in the medical plan
• Nonstate groups employees and their spouses and dependents age 18 or older who are enrolled in the medical plan
