Who is Eligible to Participate in the Health Screening, Coaching & PHA?

• Employees and spouse/dependents 18 years or older who are enrolled in the state medical plan
• Benefits eligible employees who have waived coverage in the medical plan
• Retirees and spouse/dependents age 18 or older who are enrolled in the medical plan
• Nonstate groups employees and their spouses and dependents age 18 or older who are enrolled in the medical plan


New Hires or New Plan Members: If you receive an error message about eligibility, contact Judy Crawford at judy.crawford@khpa.ks.gov or call 785-368-6341. She can verify your eligibility and help you get registered.
