HEALTHQUEST Health Coaches provide health care information and support to individuals to help them manage their chronic medical conditions and make the best health care decisions for themselves and their families. The coaching service is voluntary, private, and available 24/7/365.  Services are available at no charge to eligible participants.

Health Coaches . . .

  • Provide support for decisions you need to make about significant medical events by providing unbiased information about treatment options
  • Help you understand chronic conditions and identify effective ways to proactively manage your health
  • Offer a personalized approach that considers your individual situation and supports many medical conditions
  • Provide a single source of support for you and your family
  • Provide you with tools to be an active participant in managing your health care, including telephone, online and video resources

HEALTHQUEST Health Coaches are specially trained health care professionals such as nurses, dietitians and respiratory therapists. A Health Coach can provide information to help you make important decisions about your health, understand more about treatment choices and prepare for your doctor appointments.  Health Coaches can work with you on a wide range of health care concerns, from deciding whether to have surgery to learning how to better manage an ongoing condition such as diabetes or asthma.  Health Coaches won’t replace your doctor or other health care providers; rather, they help you get the most out of every office visit.

HEALTHQUEST Health Coaches offer support and information for many medical conditions, such as asthma, back pain, breast cancer, coronary artery disease, depression, diabetes, fibroids, heart disease, high blood pressure, kidney failure, osteoarthritis, and prostate cancer.

Use a HEALTHQUEST Health Coach when . . .

  • You’ve just been diagnosed with a condition and you want more information about the treatment choices
  • You have a doctor’s appointment to talk about treating your condition and you’re not sure exactly what questions to ask your doctor
  • You have diabetes and want to get more information about managing your diet to better control your blood sugar
