Important Things Eligible Participants Need to Know about the Gift Card Incentive

  • If you have not yet completed the online Personal Health Assessment (PHA), you must do so by September 30, 2008, to receive your $50 Gift Card.  To complete your PHA, please visit http://www.khpa.ks.gov/healthquest/pha.htm.  

  • If you have already completed the PHA, the deadline for redeeming a gift card to your choice of retailers is October 31, 2008, otherwise a gift card will be chosen for you.  To redeem your gift card, please call toll free
    1-800-390-4950.

  • The Gift Card Award is taxable to those who receive it.  For State employees and their eligible dependents,
    the estimated taxes on the award will be deducted from your paycheck.  Other members of the KHPA Health Plan (such as retirees and Non State participants), will need to declare the value of the award as miscellaneous
    income on your Federal and State 2008 tax returns, although there will be no 1099 issued to you.
