Q: How do I enroll as a Kansas Medical Assistance Program (KMAP) Provider?

A: You may access the application forms to enroll as a KMAP Provider on the Kansas Medical Assistance Program web site: Click on the Provider button on the left side of the page and the click on Enrollment Forms. The phone number for our Provider Enrollment number is 785-294-5914.

Q: How do I get help with filing a claim?

A: You may call the KMAP Customer Service Center at 1-800-933-6593 for assistance with filing a claim. The Center can also assist in resolving other claims issues.

Q: Where can I find provider manuals?

A: On the KMAP web site, Click on the Provider button on the left side of the page and then click on Manuals. Click in the “Manual Type” box to drop down a list of KMAP provider manuals.

Q: Where can I find a fee schedule?

A: Please call the KMAP Customer Service Center at 1-800-933-6593 to obtain a fee schedule. In the future, we plan to have the fee schedule as a downloadable file.
