Q: How do I apply for HealthWave?

A: Applications can be downloaded and printed from your computer by clicking here or you may call the HealthWave Clearinghouse at 1-800-792-4884 and ask to have an application mailed to you. There is also an application available online. Click on the “Apply for Services” button on the left side of the screen. Application forms are available at any Regional SRS office and many SRS Access Points in the community.

Q: Who do I call if I don’t understand what I need on my HealthWave application?

A: Call the HealthWave Clearinghouse at 1-800-792-4884, they can answer any questions you have about your application.

Q: How long must I live in Kansas before I qualify for Medicaid or HealthWave?

A: There is no set amount of time a person must live in Kansas in order to meet residency requirements to qualify for medical assistance. You must be residing in the state voluntarily, and not for a temporary purpose, meaning you must not be planning to leave the state when you apply.
